Registration Requirements

  2. Valid ID : One (1) government-issued identification card such as Philippine passport, driver’s license, Social Security System (SSS), PhilHealth, Pag-Ibig, TIN ID, Postal ID, Senior Citizen’s ID or Overseas Employment Certificate.
  3. Proof of Address : Utility bills in your name (e.g., electricity, water, gas) and a photocopy of the front page of your bank account passbook or credit card statement. If you have just moved to the Philippines, please provide proof of address from where you live.
  4. Passport Copy (for non-Filipinos): Photocopies of both sides of your passport.
